What is a Support at Home?

Support at Home are part of a government-funded program for aged care.  They are for elderly Australians who need help staying independent at home.

If you are aged over 65 and you do not want to move to a nursing home, this program may be the answer. 

​

Support at Home offer long-term support and various aged-care services including Physiotherapy.

What are the levels of Care in a Home Care Package?

​My Aged Care funds for different Support at Home Levels.

1. Level 1 - Basic Entry Level. This level of care consists of basic care services for elderly people who need the lowest level of support. It offers tailored access to essential health, social, and well-being services.

2. Level 2 - Low-Level Care Needs. This level of care is specific for seniors or people with disabilities and special needs who need long-term support. It provides aged care services and support such as nurses, personal assistances and social workers.

3. Level 3 - Intermediate Care Needs. This level similar to level 2, is specific for seniors or people with a disability or special needs who have more complex health conditions and have a greater functional limitation.

4. Level 4 - High-Level Care Needs. This level is the highest level in Australia and is designed for people who need a significant amount of complex care to continue living in the comfort of their own homes.

How does a Support at Home fit in with Flexile Physiotherapy?

Once you are approved for Support at Home funding, you will sign up with an approved support at home provider who will assist you with managing your funds on your behalf. The provider will assess your care needs and then source a provider to help provide the care you need (for example, Flexile Physiotherapy).

When you have a Support at Home Plan, we would then obtain the Support at Home Case Manager details and call them on your behalf to introduce our service. Your Support at Home Case Manager will then send us an agreement for our services or approve the physiotherapy sessions for you.

Once this occurs, we will then contact you to schedule your initial assessment, where we will assess and listen to your needs and come up with a plan to address these needs together. After the Initial Assessment, your physiotherapist will create a detailed goal plan that includes:

• A specific meaningful goal that you want to achieve

• A plan on how to achieve this goal

• Number of sessions along with a review date of when you should hopefully achieve this goal.

We will send this plan to your Support at Home Case Manager for approval that way everyone involved understands what is happening and costs involved.

Once the plan is approved, we then send an invoice to the Support at Home Case Manager for payment.
